    ​Hi Mark,

    We manage these through our asset management system Maximo. It has a mobile application called 'Plant Faults' in it you can enter all sorts of information regarding the fault, i.e. result of an accident, is this a safety concern and the technician entering the data can nominate a priority of the fault. P1 meaning that the machine is stood down immediately, through to, P4 meaning that the machine is still working fine and can be repaired when not in use. We get these emailed through from the workshop or site, and then assign WO's directly to the fault so they are related records. The system will complete both records once the WO is completed.

    We utilise the same issue for vehicle recalls, until the vehicle has been remedied by the OEM.

    Part of our process to make sure the guys in the field don't circumvent the whole thing is that they cannot order parts for a corrective maintenance job without a plant fault number, which is system generated when the technician entering the fault in the first place.

    I am sure most if not all asset management systems would have something similar, it'd be a case of looking at your current practice, what your system can do to assist and working on a process to tie the whole thing together.
